* what is the "CRAO"???
-Central retinal artery occlusion (CRAO) is a disease of the eye where the flow of blood through the central retinal artery is blocked (occluded).
There are several different causes of this occlusion; the most common is carotid artery atherosclerosis.
*what is the cause???
-The most common cause for CRAO is carotid artery atherosclerosis.
-In patients of 70 years of age and older, giant cell arteritis is more likely to be the cause than in younger patients.
- Other causes can include dissecting aneurysms and arterial spasms.
*diagnosis"التشخيص"
1-sudden painless unilateral loss of vision
"Counting finger to light perception in 90% of cases" perceeded by amaurosis fugax "episodes of transient loss of vision"
-هنلاقي المريض بيقول انه فجأة لقى نفسه مش شايف غالبا في عين واحدة غير مصحوب بألم و أحيانا مسبوق بنوبات متقطعة من عدم الرؤية.
2-marcuz -gun pupil
"No light reflex in diseased eye &normal consensal reflex"
-عند توجيه الكشاف للعين المصابة يلاحظ عدم استجابتها للضوء مع حدوث استجابة في العين الاخرى في نفس الوقت.
3-history of thrombo -embolic disease ...ex:- "AF"
-غالبا المريض عنده مشكلة قد تؤدي لحدوث جلطات مفاجئة مثل ارتجاف الاذين
*emergency management...إزااااي نلحق الحالة دي ونساعدها؟؟؟؟؟
**تحويل الحالة بسرعة جدا إلى اقرب مستشفى واثناء ذلك ممكن نعمل بعض الاجراءات اللي ممكن تساعد الحالة دي
1-ask the patient to breathe into a bag as the increase of carbon dioxide CO2 the vadodilatation occur
-نطلب مة المريض انه يتنفس في كيس فكلما زادت نسبة ثاني اكسيد الكربون كلما حدث توسع في الاوعية الدموية وكان مرور الدم بها اكثر واسهر
2-occular massage
-عمل تدليك للعين
3-diamox tab
Contains"acetazolamide 250 mg " which is a carbonic anhydrase inhibitor
-هياخذ قرصين الدياموكس مرة واحدة فقط
4-aspirin tab
-هياخد اربع اقراص مع بعض
5-sublingual dinitra tab
Contains"isosorbid dinitrate 20mg"
